Service: Medicaid Service Coordination (MSC)Purpose:
Provide services to adults and children diagnosed with a developmental disability.
Activities fall into three general categories:
1. individualized service planning,
2. advocating, and
3. maintaining records.
Responsibilities include required home visits with consumers, understanding person-centered planning, consumer directed services, and self-advocacy.
Method:
A Medicaid Service Coordinator works with individuals receiving services to provide the following:
1. Recognizing and addressing health and safety issues.
2. Facilitating Individual Service Plan meetings and other meetings as needed.
3. Communicating (verbally and in writing) with individuals, families, advocates and providers.
4. Knowledge about services and supports - traditional OMRDD funded and community based resources.
5. Assessing crisis, assisting in negotiating and resolving conflicts.
6. Accessing entitlements and benefits.
7. Advocacy - intervening, assisting and communicating on behalf of the consumer.
8. Maintaining an up-to-date Individual Service Plan/life plan.
9. Keeping a written record, including required documentation.
