Service: Family Support Service - Family ReimbursementPurpose:
To provide reimbursement funds, not funded by other sources, to people with developmental disabilities whom live with their families or guardian. The goal is to maintain the eligible consumer in their home setting by providing assistance with support services such as respite care, assistive equipment, dietary requirements and other specialty items that will decrease difficulties in daily living arrangements.
Method:
1. Coordinate and accept applications from anyone in Niagara and Erie counties.
2. Give priority to Native American Indian families for the first half of the calendar year.
3. Reviews each application for eligibility, documentation of need and to assure that the request is for funding of the last resort.
4. Prepares applicants' application information to be reviewed by the Review Committee, which is comprised of the Native American Indian Council representatives.
5. Check with the DDSO Family Reimbursement Coordinator for a Consumer Check of the approved applications.
6. Notification of decision is sent to the applicant.
