JOB OVERVIEW

JOB TITLE: Healthy Living Specialist

DEPARTMENT: Healthy Living Department

LOCATION: Buffalo, Niagara Falls or Cheektowaga offices (Flexible to one of these locations)

DIRECT SUPERVISOR: Healthy Living Coordinator


GENERAL JOB DESCRIPTION

Provide services contracted through the Western New York Integrated Care Collaborative (WNYICC) such as but not limited to: Healthy Ideas, Falls Prevention program, Caregiver program, Community Health Coaching Program, and Evidence Based Practice Workshops.


SPECIFIC DUTIES & RESPONSIBILITIES

  1. Communicate and work collaboratively with Western NY Independent Living (WNYIL) family of agencies to support coordinated service delivery for WNYICC Initiatives.
     
  2. Promote and express Independent Living (IL) philosophy.
     
  3. Provide in home services to referred consumers.
     
  4. Conduct initial screenings using the screening tool for each WNYICC program.
     
  5. Provide on going coaching of consumers in accordance to their enrolled program model.
     
  6. Ensure services adhere to the evidence-based service models.
     
  7. Ensure eligibility of consumers for services.
     
  8. Ensure all services are accurately documented in WNYIL’s Electronic Consumer Record (ECR) within two (2) working days of the date of service.
     
  9. Ensure documentation is entered into WNYICC’s portal according to their processes.
     
  10. Remain knowledgeable in WNYICC service models and the research that demonstrates their effectiveness.
     
  11. Complete training for WNYICC initiatives and act as lead trainer for service delivery.
     
  12. Conduct outreach and engagement activities to increase referrals and community supports.
     
  13. Attend Agency’s in-service training and staff meetings as well as any other agency related activities as required.
     
  14. Participate in maintaining a neat, clean and safe work environment.
     
  15. Perform other job-related duties as assigned by the supervisor.

EDUCATION & TRAINING

Must possess a bachelor’s degree in one of the following areas: Child & Family Studies, Community Mental Health, Counseling, Education, Nursing, Occupational Therapy, Psychology, Recreation, Recreation Therapy, Rehabilitation, Social Work, Sociology or Speech and hearing.


SKILLS & ABILITIES

  • Comfortable discussing suicidal ideations.
     
  • Comfortable working with consumers in a variety of settings.
     
  • Must have a valid driver’s license and vehicle in order to travel to all WNYIL Agencies.
     
  • Ability to work independently at WNYIL Agencies and off-site visits to provide seamless coordination of services.
     
  • Must have excellent verbal and written communication skills, good organization and analytical skills.
     
  • Must have intermediate level of knowledge of computers including the use of Excel spreadsheets, databases, word processing and networks.
     
  • Perform in a professional, conscientious, and efficient manner for the purpose of ensuring a better quality of life for persons with disabilities.

WORKING CONDITIONS

WORK ENVIRONMENT: Office / Community

PHYSICAL DEMANDS OF THE JOB: Sitting for extended periods of time.
Occasional lifting of not more than 15 pounds.

HOURS / SHIFTS: 37.5 – 40 hours per week. M – F 8:30 a.m. – 5:00 p.m.
Overtime rarely required.

CLASSIFICATION: Full-time, non-exempt, Professional staff.
Range: $49,000 - $55,000
