Services
Transition Services or Bridging means that a consumer may be looking to move from an institutional setting to the community. Institutional settings include long term care facilities, hospitals, psychiatric centers, and residential treatment centers/facilities.
Open Doors
Open Doors is a program that supports individuals residing in nursing homes or intermediate care facilities in accessing the necessary care and services in their home and community. Transition Specialists assist with various aspects such as identifying required services, finding an affordable and suitable home, arranging in-home assistance, and preparing for a successful transition back to the community.
Adult Transitional Services
Open Doors supports the consumer in the nursing home transition, but an Independent Living Specialist (ILS) can support a consumer in obtaining services to remain in the community. Services can include but are not limited to obtaining Consumer Directed Personal Assistance Program (CDPAP) Aide Services, accessing Waiver Services, reviewing Managed Care Organizations (MCO) or Managed Long-Term Care (MLTC) Services, and connecting to the Department of Social Services (DSS).
